Seniors and individuals with disabilities (and their families) often don't know where to turn when they need help. Our Link-Age department's Options Counselors work closely with our Case Managers to coordinate services that will help these individuals continue living on their own for as long as possible.

This team of professionals is here to help guide you through the system so you get the help you need. We are committed to linking our clients to community services through local providers and also monitoring those services. Our professional Case Managers will:
