Senior Care Consultant: A new endeavor at Generations
The Senior Care Consultant Program is a comprehensive concierge service intended to help individuals, families, and professionals navigate the complicated elder care network, locating and arranging services to meet the complete array of needs that arise as people age. With the knowledge and expertise our program can bring to your situation, we can truly be your partners in collaboration, coordination and caring to plan for and monitor your loved one’s care.
This program will has three main components:
- Enhanced Discharge Planning – for better coordination of care to patients with a recent hospital discharge.
- Dementia Care Consulting – for families, long-term care facilities, medical practices and health care providers.
- Family Care Consulting – for individuals needing care, families, workplaces needing to reduce lost work time.
Program Mission: The Senior Care Consultant Program will offer a comprehensive approach to meeting the needs of individuals and entities facing the challenges of planning for senior care.
